MORTUARY ASSISTANCE PROGRAM

OBJECTIVES:

1. To provide financial or any material assistance as an expression of moral support to the surviving family members of any alumnus/alumna who is deceased.
2. To provide financial or any material assistance as an expression of moral support to the suriviving family members of any HCDC employees, currently employed, or retired from services as a gesture of appreciation of his service for the education and development of the alumni who once have been under his tutelage.
3. To provide financial or any material assistance as well as any expression of moral support to the surviving family members of any student or pupil who may die during the current school year he/she is enrolled in HCDC.
   

COVERAGE:

1. Any individual who graduate from HCDC: Grade School, High School, TVET, College and Graduate School whose name appears in the list of alumni from 1956 to the present.
2. All employees: teaching and non-teaching staff, administrators, part-time faculty members who are currently employed with HCDC at the time of death.
3. Retired employees of HCDC.
4. Spouse, children, parents of an alumnus/alumna or HCDC employee.
5. Any student or pupil currently enrolled in any department at the time of his death.
6. A family is entitled to only one mortuary aid per death.
   

REQUIREMENTS/PROCEDURES:

1. Presentation or submission of death certificate.
2. Verification and certification of status as an alumnus/alumna.
3. Verification and certification may be done by the alumni coordinator of the department, or by the department head, or any reliable person/s known to any HCDC employee or administrator.
4. Submission of information to the alumni president who shall approved the corresponding amount.
5. Release of the amount; by the alumni treasurer; the alumni president and coordinator, or any other officer of the association personally hands over to the recipient the amount together with a wreath.
   

SCHEDULE OF ASSISTANCE:

1. Alumni - Php2,500.00 cash plus wreath worth Php500.00
2. Current employees - Php2,000.00 cash plus wreath worth Php500.00
3. Retired employees - Php2,000.00 cash plus wreath worth Php500.00
4. Part-time faculty - Php1,500.00 cash plus wreath worth Php500.00
5. Student/pupils - Php1,500.00 cash plus wreath worth Php500.00
6. Spouse, children, parents - Php1,000.00 cash plus wreath worth Php500.00
   

FUND SOURCING:

1. Membership fee:
   
   a. Grade School - Php5.00 from membership fee of Php25.00.
   b. High School - Php 10.00 from membership fee of Php50.00.
   c. TVET - Php 15.00 from membership fee of Php75.00.
   d. College - Php20.00 from membership fee of Php100.00.
   e. Graduate School - Php40.00 from membership fee of Php200.00.
   
   
2. Fund raising projects - 20% of the net proceeds shall be allocated to the funds of mortuary assistance program.
   
   
3. Donations from patrons, benefactors, friends and alumni.
